Insurance Information

We make every effort to provide you with the finest care and the most convenient financial options.  To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.  To minimize your out-of-pocket expense at the time of your visit, we will submit for direct payment of your dental plan benefits.  We ask you to take care of an estimated co-payment plus deductible, if applicable, at the time of service.  Upon receipt of your company’s plan payment, we will refund any credit or bill the balance to you.  If you have any problems or questions, please ask our staff.  They are well informed and up-to-date.  We can also be reached by phone at Sunnyvale Endodontics Phone Number 408-736-4669.
